We offer medical, dental, nutrition, behavioral health, a mobile health unit, and two locations for family support services. The Physician will join a dedicated team of healthcare providers to deliver comprehensive and continuing care. This will include routine and emergency clinic care, support of health promotion, and preventative medicine. This person will support CHP’s mission, vision, and values. Essential Skills and Qualifications: Qualifications: Graduation from an accredited medical school Completion of an accredited residency Licensing through the USMLE Active and unrestricted Massachusetts medical license Specialty board certification or obtaining such within one year of hire.   • CHP Physician's Assistant Mike Jefferson here for this month's Mobile Health Minute to discuss April as National Minority Health Month. This annual observance builds awareness about the health disparities that persist among racial and ethnic minority and American Indian/Alaska Native (AI/AN) populations and encourages everyone to take action to end these inequities. This year's theme, Be the Source for Better Health: Improving Health Outcomes Through Our Cultures, Communities, and Connections, is about understanding how the unique environments, cultures, histories, and circumstances (known as social determinants of health, or SDOH) of racial and ethnic minority and AI/AN populations impact their overall health. Community Health Programs is committed to providing quality, equitable, and respectful care and services that are responsive to diverse cultural health beliefs and practices, preferred languages, economic and environmental circumstances, and health literacy levels. When patients are provided with culturally and linguistically appropriate information, they are better able to create healthier outcomes for themselves, their families, and their communities.

  • As a physician dedicated to improving the lives of those with chronic health conditions, Lisa Nelson, MD, witnessed firsthand the transformative healing that happens in group health visits.   • It was a packed room full of great stories, years' worth of photos, food, fun, laughter, and tears as CHP gave a bittersweet farewell to Michelle Derr, CHP Senior VP of Family Services and Mobile Health. Former and current board members, CHP leadership, coworkers, family, and friends came to tell fun stories and talk about the significant impact Michelle has had on thousands of lives in the southern Berkshires in her 37 years at CHP. State Representative Smitty Pignatelli presented Michelle with a State Designation for her impact on the community. CHP's great Family Services staff will continue with Michelle's drive and serve the community, but things won't be quite the same without you, Michelle. We wish you a retirement full of art, grandchildren, travel, and fun. You've earned it! Thanks to the Family Services team for throwing such a great party.

  • CHP showed up to celebrate Dr. Claire Horth's win at this year's 40 Under Forty Award Ceremony at Berkshire Community College. 40 Under Forty is an annual celebration of talented people in the Berkshires, under the age of 40, who have a deep dedication to improving the quality of life for those living and working in our community. 40 Under Forty awards honor people from all areas of Berkshire County who are making a difference.
